What is the clinical significance of ScvO2 in septic shock?

ScvO2 reflects the balance between oxygen delivery and consumption. A value <70% indicates that tissues are extracting more oxygen than normal, suggesting inadequate DO2. In early goal-directed therapy (EGDT) for septic shock, Rivers et al. targeted ScvO2 ≥70% alongside MAP ≥65 and CVP 8–12 as resuscitation endpoints. While three landmark RCTs (ProCESS, ARISE, ProMISe) showed EGDT was not superior to usual care, ScvO2 monitoring remains a useful guide when clinical assessment is insufficient.

How does ScvO2 differ from SvO2 (mixed venous)?

SvO2 (mixed venous saturation) is measured from the pulmonary artery via a PA catheter and reflects whole-body oxygen extraction. ScvO2 is measured from a central venous catheter in the superior vena cava and overestimates SvO2 by approximately 5–8% because it does not include coronary and inferior vena caval blood. In clinical practice, ScvO2 ≥70% is used as a surrogate for SvO2 ≥65%, avoiding the risks of PA catheter insertion.
