What lactate clearance target should guide sepsis resuscitation?

The LACTATES trial (Jones et al., JAMA 2010) demonstrated that lactate clearance ≥10% at 2 hours was non-inferior to ScvO2 ≥70% as a resuscitation endpoint, with similar 60-day mortality. Current Surviving Sepsis Campaign guidelines recommend targeting lactate normalization ( 4 mmol/L) despite resuscitation carries >40% mortality.

Can elevated lactate always be attributed to tissue hypoperfusion in critical illness?

No. In sepsis, lactate elevation has multiple mechanisms: type A lactic acidosis from tissue hypoperfusion is the most common, but type B mechanisms include accelerated aerobic glycolysis stimulated by beta-2 adrenergic activation (epinephrine effect), mitochondrial dysfunction, liver failure reducing lactate clearance, and thiamine deficiency impairing pyruvate dehydrogenase. Empirical thiamine supplementation should be considered in patients with refractory hyperlactatemia, especially those receiving high-dose norepinephrine.
