How does SVV compare to PPV for predicting fluid responsiveness?

SVV and PPV both predict fluid responsiveness with comparable AUC (~0.84–0.94) in appropriate patients. PPV is easily measured from any arterial line. SVV requires a continuous cardiac output monitoring device (PiCCO, LiDCO, FloTrac) that derives stroke volume from arterial waveform analysis. Both share the same physiological basis (heart-lung interaction during positive pressure ventilation) and have the same limitations regarding ventilation mode and tidal volume.

Can SVV be used in spontaneously breathing patients?

No. SVV (and PPV) require controlled mechanical ventilation with passive chest mechanics. Spontaneous respiratory efforts create unpredictable and variable changes in intrathoracic pressure, making SVV unreliable as a preload indicator. In spontaneously breathing patients, passive leg raise is the preferred dynamic test. Some devices offer respiratory variation in plethysmographic signal (PVI) as a non-invasive alternative, though its accuracy in spontaneously breathing patients remains debated.
