How is PPV calculated from an arterial line?

PPV (%) = (PPmax − PPmin) / [(PPmax + PPmin) / 2] × 100, calculated over one respiratory cycle. PPmax is the maximum pulse pressure (systolic minus diastolic) during inspiration; PPmin is the minimum pulse pressure during expiration. Automated calculation is available on most modern monitors. Manual calculation from screenshot printout requires identifying the highest and lowest pulse pressure values within a single respiratory cycle.

What are the limitations of PPV in clinical practice?

PPV is only valid under strict conditions: fully controlled mechanical ventilation, tidal volume ≥8 mL/kg PBW, heart rate/respiratory rate ratio >3.6, absence of arrhythmias, and no significant right ventricular dysfunction. Low tidal volume ventilation (4–6 mL/kg) used in ARDS reduces intrathoracic pressure variation, making PPV unreliable — PPV may be low even in fluid-responsive patients. In these scenarios, passive leg raise or end-expiratory occlusion testing is preferred.
