Cryptogenic (now 'unknown') etiology, normal development before spasm onset, early diagnosis and treatment (<4 weeks from onset), and rapid EEG response (hypsarrhythmia resolution within 2 weeks) are associated with favorable neurodevelopmental outcomes.

What is first-line treatment for infantile spasms?

ACTH (high-dose) or vigabatrin are first-line. UKISS trial showed combined ACTH + vigabatrin superior to either alone for hypsarrhythmia resolution. Tuberous sclerosis patients respond particularly well to vigabatrin monotherapy. ACTH/vigabatrin should be started within 2 weeks of diagnosis.
