Which patients are appropriate candidates for dynamic fluid responsiveness assessment?

Dynamic tests (PPV, SVV, passive leg raise) are most accurate in patients with: controlled mechanical ventilation, tidal volume ≥8 mL/kg, sinus rhythm (not atrial fibrillation), and no significant right heart failure. PPV and SVV lose accuracy with spontaneous breathing efforts, arrhythmias, and tidal volumes <7 mL/kg (common in lung-protective ARDS ventilation). Passive leg raise is applicable regardless of ventilation mode or rhythm.

What is the threshold for predicting fluid responsiveness?

A positive fluid response (≥10–15% increase in cardiac output/stroke volume after 500 mL fluid challenge) is predicted by: PPV >13%, SVV >13%, passive leg raise-induced cardiac output increase ≥10%, or end-expiratory occlusion test cardiac output increase ≥5%. In a 2016 meta-analysis (Marik et al.), PPV had AUC 0.94 for predicting fluid responsiveness in appropriate patient subgroups — but only ~50% of all ICU patients are actually fluid-responsive.
