What are the KDIGO AKI staging criteria?

Stage 1: Creatinine ×1.5–1.9 times baseline within 7 days, OR absolute rise ≥0.3 mg/dL within 48h, OR UO <0.5 mL/kg/h for 6–12h. Stage 2: Creatinine ×2–2.9 times baseline, OR UO <0.5 mL/kg/h for ≥12h. Stage 3: Creatinine ×3 times baseline, OR creatinine ≥4 mg/dL, OR initiation of RRT, OR in patients <18 years, GFR decrease to <35 mL/min/1.73m², OR UO <0.3 mL/kg/h for ≥24h or anuria ≥12h.

What interventions should accompany KDIGO staging?

KDIGO guideline recommendations by stage: Stage 1 — identify and treat cause, optimize volume status, avoid nephrotoxins, review medications, monitor UO and creatinine 8-hourly. Stage 2 — all Stage 1 measures plus consider ICU admission, nephrology consultation, and evaluate for RRT initiation criteria. Stage 3 — all Stage 2 measures plus initiate RRT if indicated (refractory acidosis, hyperkalemia, uremia, or fluid overload unresponsive to diuretics). Avoid unnecessary nephrotoxin use across all stages.
