When is apixaban 2.5 mg BID used instead of 5 mg BID in AF?

Dose reduction to 2.5 mg BID when 2 or more of: age ≥80 years, body weight ≤60 kg, serum creatinine ≥1.5 mg/dL. One criterion alone does not trigger reduction. In VTE treatment, full-dose 10 mg BID × 7 days then 5 mg BID regardless of these criteria (renal adjustment not required unless severe: CrCl <25 mL/min).

Does apixaban require dose adjustment for severe renal impairment?

For AF: use standard dose reduction criteria (not eGFR alone). For VTE: avoid if CrCl <25 mL/min. Apixaban is not recommended in dialysis for AF by FDA labeling (limited data). AHA/ACC 2023: apixaban 5 mg BID may be used cautiously in ESRD/dialysis for AF based on emerging safety data, though not approved for this indication.
