How is delta SOFA used in sepsis identification?

Sepsis-3 uses SOFA increase of >=2 from baseline as part of the infection-related organ dysfunction definition. Baseline SOFA assumed 0 in community-acquired infections. In patients with pre-existing organ dysfunction (e.g., CKD, cirrhosis), baseline SOFA must account for chronic impairment to avoid overestimating acute change.
