How is enoxaparin dosed for DVT/PE treatment?

Enoxaparin 1 mg/kg SC every 12 hours OR 1.5 mg/kg SC once daily (inpatient). Use actual body weight. Renal adjustment: reduce to 1 mg/kg once daily if CrCl 40 or weight >150 kg), use anti-Xa monitoring 4 hours post-dose (target 0.5–1.0 IU/mL for BID dosing).

When should anti-Xa be monitored with LMWH?

Routine monitoring not recommended in average-weight adults with normal renal function. Monitoring indicated for: extremes of weight (BMI >40 or <60 kg), CrCl 15–30 mL/min, pregnancy (anti-Xa target 0.6–1.0 IU/mL for treatment BID dosing), mechanical heart valves, and children. Draw 4 hours after SC injection.
