How accurate is MDRD vs CKD-EPI?

CKD-EPI 2021 is more accurate at eGFR >60 mL/min/1.73m². MDRD tends to underestimate GFR in normal and mildly reduced kidney function.

When is MDRD still used?

MDRD remains in use in labs that have not updated to CKD-EPI 2021, and for research comparison with historical cohorts.

eGFR = 175 × (Cr)^-1.154 × (Age)^-0.203 × 0.742 (if female) × 1.212 (if Black, now removed in updated practice).
