SSC 2021: MAP target 65 mmHg for most septic shock patients. Higher target (80 to 85 mmHg) in chronic hypertension (prior RAS disease) may reduce AKI (SEPSISPAM trial). Lower target 60 to 65 mmHg acceptable in young patients without cardiac/renal disease (65 TRIAL). Individualized MAP targets based on baseline BP and organ response.

How are vasopressor doses compared across agents?

Vasopressor equivalence (approximate): norepinephrine 0.1 mcg/kg/min is approximately equal to dopamine 10 mcg/kg/min, epinephrine 0.1 mcg/kg/min, phenylephrine 1.0 mcg/kg/min. Norepinephrine is first-line in septic shock (superior to dopamine in SOAP II trial: lower arrhythmia rate, lower mortality in cardiogenic shock subgroup). Vasopressor equivalent score = sum of vasopressor doses normalized to norepinephrine.
