Which phosphate binder is preferred for CKD patients?

Non-calcium binders (sevelamer, lanthanum) are preferred in CKD G3b-G5D to avoid calcium loading, particularly with vascular calcification. Calcium-based binders are acceptable if hypercalcaemia is absent.

How is sevelamer dosed?

Sevelamer: start 800-1600 mg three times daily with meals. Titrate by 800 mg/meal every 2 weeks. Doses range from 800-7200 mg/meal based on serum phosphate level.

What serum phosphate target is recommended?

KDIGO suggests maintaining serum phosphate in the normal range (2.5-4.5 mg/dL) for CKD G3-G5 and closer to normal (3.5-5.5 mg/dL) for dialysis patients.
