What does persistent hyperlactatemia indicate in septic shock?

Lactate >4 mmol/L after 6 hours of resuscitation: poor prognostic sign (in-hospital mortality >50%). Causes: tissue hypoperfusion, microcirculatory failure, mitochondrial dysfunction, impaired hepatic clearance, increased glycolytic flux (Warburg effect), epinephrine effect on beta-2 receptors. Serial measurement more informative than single point.
