ASHP/IDSA 2020 guidelines recommend AUC-guided dosing over trough-only monitoring. For programs still using trough-based monitoring: serious infections (bacteremia, endocarditis, pneumonia, meningitis) target trough 15–20 mg/L; uncomplicated skin/soft tissue 10–15 mg/L. Trough >25 mg/L predicts nephrotoxicity.

How often should vancomycin levels be checked during therapy?

Steady-state trough with traditional dosing: draw before 4th or 5th dose. Recheck every 2–3 days for stable patients or with any significant change in renal function. Daily monitoring required for critically ill patients or those with AKI. Trough-only monitoring is being phased out in favor of AUC methods.
