How is the ABSI score calculated and what does each component contribute?

ABSI scores five factors: Age (1 point per decade over 20, capped at 5); Sex (1 point for female); Inhalation injury (1 point if present); Full-thickness burn (1 point if present); Total body surface area burned (0 = <10%, 1 = 10–19%, 2 = 20–29%, 3 = 30–39%, 4 = 40–49%, 5 = 50–59%, 6 = 60–69%, 7 = 70–79%, 8 = 80–89%, 9 = ≥90%). Total ABSI: ≤2 = very low threat to life; 3–4 = moderate; 5–6 = moderate-severe; 7–8 = severe; 9–10 = very severe; 11 = maximum threat.

What fluid resuscitation formula is used for major burns in the ICU?

The Parkland formula remains the most widely used initial resuscitation guide: 4 mL × weight (kg) × TBSA% burned of Ringer's lactate in the first 24 hours, with half given in the first 8 hours (from time of injury, not time of admission) and the remaining half over the following 16 hours. Modified Brooke (2 mL/kg/TBSA% RL) is an alternative. Fluid resuscitation should target urine output of 0.5–1 mL/kg/h. Colloid adjuncts (albumin 5%) are added at 8–12 hours in burns >40% TBSA.
